Patient and Family Advisory Council

Thank you for your interest in applying to the Patient and Family Advisory Council (PFAC) at Crawford County Memorial Hospital.

The CCMH PFAC is made up of current and former patients, family members, and hospital staff who come together to support and enhance patient care. This 12-member council plays a vital role in guiding improvements at CCMH by offering valuable insights on patient care, safety, quality, and communication.

Council Goals:

  • Offer valuable insight to hospital leadership and staff about the needs and expectations of patients and their families
  • Collaborate with staff to enhance services and processes
  • Promote improvements in patient safety, care quality, and satisfaction
  • Serve as a voice for patients and families in planning new programs and care areas
