Meet Your CCMH Health Navigators

Navigating the complexities of healthcare can feel overwhelming. From understanding insurance plans to finding the right resources, it’s easy to feel lost. But what if you had a dedicated guide to help you through it all? At CCMH, we’re proud to have health navigators like Heather DeLance and Lexi Fineran, who act as guides for our patients and community members.

What is a health navigator?

According to DeLance, navigators are the go-to experts for understanding the healthcare landscape and accessing the support you need.

“Our role involves assisting patients, particularly those with Medicare, by working closely with nurses and doctors. We are a crucial link in ensuring patients receive comprehensive care and have access to essential resources.”

Fineran added, “Our focus on helping patients understand their insurance better is a mission that can often feel like deciphering a foreign language. We help patients feel better about their coverage and their care.”

How can a Health Navigator help?

Health navigators undertake varied tasks and are incredibly impactful, Fineran explained.

“Imagine facing a sky-high prescription cost or needing a specialist referral but not knowing where to turn. This is where we, as health navigators, step in to help. We  research and connect patients with resources to lower medication expenses, find referrals closer to home, and ultimately alleviate financial burdens. Many patients find comfort in receiving care within familiar surroundings, and navigators help bridge that gap.”

What about routine screenings?

Beyond financial assistance and referrals, DeLance says a significant part of their role involves proactive care through scheduling crucial screenings.

“From diabetic eye exams and colonoscopies to mammograms, Lexi and I actively reach out to patients to explain the importance of these preventative measures. Discussing screenings like colonoscopies can be met with hesitation. It’s our job to help educate people on why these tests are vital for their long-term health and even explore alternative options when necessary.”

Can a Health Navigator help me save money?

DeLance also stated that the financial impact of their work is tangible.

“For example, we helped a patient burdened with $6,000 in medical bills from surgeries. By assisting them in applying for a hospital assistance program, they were able to get the entire amount covered for the remainder of the year, providing immense relief and peace of mind.

Fineran added, “Both of us have experienced instances of helping patients, both Medicare and non-Medicare, access lower-cost medications through various avenues, including coupon cards. It is our goal to help find solutions for everyone.”

How do I access their support?

Understanding how to access their support is crucial. Both DeLance and Fineran noted they work  closely with care teams and often connect with patients during their visits.

“Patients can easily reach out to either of us. Whether it’s simply walking in the CCMH Medical Clinic and asking for a health navigator or calling the Clinic line (712-265-2700), our doors are open. We encourage patients to reach out with any concerns or questions they may have.”

Empathy and Understanding

Recognizing that discussing healthcare concerns, especially financial difficulties, can be sensitive, DeLance and Fineran prioritize empathy and understanding.

“We both feel it is our responsibility to create a safe and comfortable space for patients to share their worries. We’re here to listen without judgment and to advocate for their needs.”

Fineran also pointed out, “We’re not here to judge anyone. We’re just here to help you. Our role is to act as an intermediary between patients and providers, ensuring that every voice is heard, and every need is addressed.”

DeLance offered her reassurance, “You empathize with patients. They are people just like we are. We want you to know that you’re being heard, you’re being cared about. Our commitment is going the extra mile to assist anyone in need.”
