Acute Care
Acute care describes the level of treatment received in the Medical (inpatient) unit at Crawford County Memorial Hospital. The hospital’s highly trained nursing staff cares for patients of all ages with a wide variety of health concerns. Acute care nurses are trained in basic life support, advanced cardiac life support and pediatric advanced life support.
Patients who are in need of acute care stay in spacious, private rooms complete with private bathrooms.
Acute Hospice Care
Hospice is a program that provides special care and support for people nearing the end of life and their families. Hospice focuses on patient comfort and quality of life rather than cure. Local hospice services are facilitated through Crawford County Home Health, Hospice & Public Health, St. Croix Hospice, or Gentiva Hospice.
Crawford County Memorial Hospital works in collaboration with Crawford County Home Health, Hospice & Public Health, St, Croix, and Gentiva to provide general inpatient care also known as acute Hospice care. Hospice patients can be admitted to the hospital when pain or symptom management can no longer be managed in the client’s home setting which also includes the nursing home.
Crawford County Memorial Hospital has two extra-large, private rooms available for Hospice patients. These rooms are located at the end of the hallway of each patient care wing and feature a semi-private sitting area for family directly across the hall from the patient room. When a Hospice client is admitted, he/she will continue to have daily assessments from his/her hospice nurse. Other hospice caregivers will come as needed. In addition to care provided by hospital nursing staff, the patient will also be visited daily by his/her physician.
Family members of Hospice patients have unlimited visiting hours and may stay overnight in the room with their loved ones. Homeward Healing: Transitional Care Services at CCMH
When you leave the hospital following treatment of an acute condition, you may require ongoing care for a chronic medical condition, as well as assistance with your day-to-day routine. Your care may necessitate special equipment or services. In these situations, a transitional care — or “swing bed” — unit provides supportive care and promotes safe and timely passage across care settings.
At Crawford County Memorial Hospital, we are proud to offer Homeward Healing, a short-term (less than 21 days) transitional care service designed for medically complex patients transitioning from the hospital to home or from one care setting to another. The goal of Homeward Healing is to assist in recovery by providing the nursing and rehabilitative care necessary to help you or your family member regain a certain level of independence.

High-quality transitional care is especially critical for older people with complex or chronic conditions and for their family caregivers. To qualify for admission to our transitional care unit, patients must be able to participate in daily intensive rehabilitation therapy with the ultimate goal of restoring their maximum level of functioning and fitness.
