Medical Records Release of Information
A Medical Release of Information (ROI) Form is used to request that a health care provider release a patient’s medical records, either to the patient, a third party (such as an employer or insurance company), or both. There are several common reasons for a release of information, including for medical treatment purposes, medical billing, insurance billing, health studies, legal proceedings, and marketing purposes.
The Health Information Portability and Accountability Act (HIPAA) has rules in place to protect health information from being improperly used or disclosed. When someone requests access to your medical records, HIPAA requires that they obtain your consent. Using a Medical ROI Form helps prevent the release of medical records to unauthorized parties and helps keep your information confidential.

Becoming A Patient
Preparing For Your Appointment
Each time you visit Crawford County Memorial Hospital for a prescheduled appointment, you will need to “check in” at the registration desk for admission. To assist in speeding up the check in process, you should bring the following items:
- The patient’s social security number
- Current insurance card
- Employment information of the insured
- Valid photo ID
- List of medications the patient is currently taking
- Name of primary care provider
- If the patient has an advanced directive, be sure to bring a copy of this along for file purposes.
Additionally, please contact your insurance company before your hospital visit to meet any pre-authorization requirements. If these requirements are not met, your benefits may be reduced, leaving you responsible for paying most or all of your hospital expenses.
Patients covered by Medicare will also need to complete the Medicare Secondary Payer Questionnaire. This Questionnaire is available at our business office.

Interpreter Services
CCMH has Spanish language interpreters available the following hours:
- Monday – Thursday: 7:00am – 7:00pm
- Friday: 7:00am – 5:00pm
- Saturday: 8:00am – 12:00pm
We offer interpreter services after hours through trusted remote services, such as Propio and Language Line. Interpretation can be provided by phone or video to help you communicate clearly with your care team.

- In-Person: Request access with one of our Patient Access representatives at your next appointment.
- Online: Click this link and email us at info@ccmhia.com.
Follow the Instructions
You will receive email instructions within 48 hours to complete your setup. The email with the link will come from noreply@iqhealth.com.
Login to Your Account
Once registered, use your credentials to log in and start managing your healthcare information.

Mobile App: Download the HealtheLife app (Cerner Corporation) for easy access on your smartphone or tablet.
Once you have downloaded the app, search for CCMH HealthLife Portal.
The Patient Portal is a free service available to all patients of the CCMH health system. It is secure, confidential, and available 24 hours a day from anywhere with an internet connection. View it from your home computer or your mobile device.
From your Patient Portal account, you can see many things that can be found in your medical record. Available items include a list of your medications, allergies, immunizations, lab results, vitals, and recent visits.
If you feel that information provided in your Patient Portal account is incorrect, please contact CCMH at 712-265-2500 as soon as possible. Members of our staff will do their best to correct the information in a timely and accurate manner.
Patients can request an appointment. After submission, one of our schedulers will contact you by phone within 24 hours to confirm your appointment or suggest a better time. Any requests made on Friday evenings or on weekends won’t be answered until the following Monday. When requesting your appointment, remember to schedule at least 48 hours from the current time.
Information from your visits is sent to your portal account at certain times throughout the day. This “information sweep” begins at midnight each night. If for some reason you feel the information is not showing after the appropriate time period, please contact CCMH Information Technology staff.
The CCMH lab has the capability to do many lab tests that are ordered by your physician. All lab work that is performed here at our facility will show in your patient portal. Because some tests require further evaluation and/or use specialty equipment, they may need to be sent out to another facility. This information is then manually entered into your medical record and will not show in your portal. If you would like a copy of lab work that does not show in your portal account, please call CCMH and ask to speak to the Health Information Management (Medical Records) staff.
At the current time, this feature is not available. In the future however, you will be able to manage your health information via your Portal.
