Diabetes Management Program

Diabetes is a disease marked by high blood sugar levels as a result of either the pancreas not making enough insulin or cells not responding to the insulin that is produced.

At CCMH Medical Clinic we have a team dedicated to helping you understand and manage your diabetes. Your diabetes care team includes a Clinical Health Coach and Registered Dietician who will work together with you and your provider.

  • Provide education about diabetes and diabetes management
  • Partner with you to set personalized goals and create a plan to meet them
  • Support you through face-to-face visits and follow-up phone calls
  • Empower you to confidently manage your diabetes

Nutrition Consultation

Crawford County Memorial Hospital’s Registered Dietitian, Brooke Tasler, works closely with our medical staff to promote better health through proper nutrition. This includes everything from reviewing daily patient and staff menus to ensure meals include balanced servings from each food group, to providing individualized nutrition counseling.

Brooke offers one-on-one guidance tailored to each patient’s condition, level of education, and personal health goals.

Beyond patient care, Brooke is also available to provide Nutrition Education for civic groups and organizations.
