At CCMH Medical Clinic we have a team dedicated to helping you understand and manage your diabetes. Your diabetes care team includes a Diabetes Care and Education Specialist and Registered Dietician who will work together with you and your provider.

- Support you through face-to-face visits and follow-up phone calls

Nutrition Consultation
Crawford County Memorial Hospital’s Registered Dietitian, Brooke Tasler, works closely with our medical staff to promote better health through proper nutrition. From reviewing patient and staff menus to providing individualized nutrition counseling, Brooke helps patients build balanced, practical eating habits that support overall wellness.
CCMH also follows the updated 2026 Food Pyramid guidelines, focusing on real, nutrient-dense foods to support long term health. Click here to learn more about the new recommendations.
Brooke offers one-on-one guidance tailored to each patient’s condition and goals and is available to provide nutrition education for community groups.
