Our Patient Portal allows you to view your test results and health records, update your personal information, and communicate securely with your healthcare providers, all from the comfort of your home.
Access the Portal
The portal contains information from June 24, 2024 onwards, please click the link below to access our Patient Portal. This portal is available through web browser, Apple devices, and Android devices.
Mobile App: Download the HealtheLife app (Cerner Corporation) for easy access on your smartphone or tablet.
Once you have downloaded the app, search for CCMH HealthLife Portal.
Register for Your Account
If you don’t have an account yet, there are two easy ways to register:
- In-Person: Request access with one of our Patient Access representatives at your next appointment.
- Online: Click this link and email us at info@ccmhia.com.
Follow the Instructions
You will receive email instructions within 48 hours to complete your setup. The email with the link will come from noreply@iqhealth.com.
Login to Your Account
Once registered, use your credentials to log in and start managing your healthcare information.
