Crawford County Memorial Hospital continues to expand access to family medicine and primary care in Denison, Iowa, as several CCMH Medical Clinic providers continue to grow their practices and welcome patients.
The CCMH Family Medicine team provides care for patients of all ages, with services ranging from preventive care and routine wellness visits to chronic disease management, women’s health, acute care, sports medicine and more. Several providers also participate in the CCMH Extended Hours Clinic, providing additional opportunities for patients to receive care outside of traditional clinic hours.
Samantha “Sam” Pauley, ARNP, FNP-C
Samantha “Sam” Pauley provides family medicine care with professional interests in women’s health, family planning, health promotion and well visits across the lifespan.
Born and raised in Crawford County, Pauley is passionate about providing compassionate, high-quality care close to home. She sees patients during regular clinic hours and through the CCMH Extended Hours Clinic.

Macklin “Mac” Conrad, PA-C
Macklin “Mac” Conrad joined the CCMH Family Medicine team in October 2025. His professional interests include sports medicine, acute care and orthopedics.
Originally from rural Iowa, Conrad values the relationships and connections that are central to rural healthcare. He sees patients during regular clinic hours and through the CCMH Extended Hours Clinic.

Kiaya Conrad provides family medicine care for patients of all ages, with professional interests in health promotion, chronic disease management and preventive care.

Cyilee Martin, DNP, FNP-C
Cyilee Martin has provided family medicine care at CCMH Medical Clinic since November 2024.
With more than 15 years of nursing experience across emergency medicine, family practice, inpatient care, hospice, cardiology and psychiatric care, Martin brings a broad clinical background to her patient-centered practice.
Martin sees patients Monday, Wednesday and Friday.
